TermLife.com LLC. is committed to the protection of your personal information. This privacy statement describes how we collect, use, protect, and store the information you provide to us. It also explains the steps you can take to review, modify, and delete your information. This statement is available on our homepage and at all points on our Web site where personally identifiable information is requested.

Children\’s Information

Protecting the privacy of children is especially important. For that reason, we never collect information on our website from those we actually know are under 13, and no part of our Web site is structured to attract anyone under 13.

Information We Collect

TermLife.com is the sole owner of the information collected on www.TermLife.com.com. TermLife.com. collects information from our users at several different points on our Web site. We only collect information that is necessary for us to provide the user with the most accurate quotes possible and to fulfill our responsibilities as the user\’s representative throughout the application process.

Quote Questionnaire

We request information from the user on our Quote Questionnaire Forms. A user must provide basic demographic information (such as state of residence, county of residence, and zip code), personal information (such as date of birth, gender, and tobacco/nicotine use), and private health information (such as current medical conditions). This information is used to provide quotes for the selected insurance products and is necessary to provide the most accurate quote possible. At no point on the questionnaire is the user required to provide personally identifiable information (such as name, social security number, address, telephone number, and email address).

Application Request

We request information from the user on our Application Request Form. A user must provide contact information (such as name, telephone number, mailing address, and email address). This information is used to complete the user\’s request for insurance applications,. The information is used to contact the user if there are problems processing requests, to follow up on applications sent, to follow up on applications received, to service the user during the application (underwriting) process, and to service the user beyond the closing of the insurance transaction.

Online Applications

Our Web site offers optional online applications (Virtual Forms) for many of the insurance products we market. We request information from the user on our Virtual Forms. A user must provide contact information (such as name, telephone number, mailing address, and email address), personal information (such as date of birth, place of birth, gender, tobacco/nicotine use, citizenship status, driving record, etc.), personally identifiable information (such as social security number and driver\’s license number), private health information (such as current/past medical conditions, current/past medications, and current/past physicians), and financial information (such as credit card number number/expiration date). This information is used to populate the insurance company\’s electronic applications.

Use of Information

All information collected is used to provide the user with quotes on insurance products, to process insurance application requests, to assist with application completion, to assist insurance companies with application processing, and to provide customer service support. We do not use information for any other reasons.

We do not sell, distribute, or otherwise make available user information to outside parties except to the extent necessary to perform the above listed duties.

Submitted Applications

Users that submit applications will receive communications from us by email, U.S. mail, and telephone. Such communications will be made on an as needed basis to assist with processing the application and facilitate the underwriting process. Examples of such communications are to seek additional/missing information, update the user as to the status of the application and to provide the user with the insurance company\’s decision and/or offer.

Communications

Users who wish to opt-out of certain types of communications may do so by sending us an email. Users must specify which type of communication they wish to opt-out of (such as telephone or email). Users must allow at least one form of communication in order for us to fulfill our responsibilities.

Sharing Information

We share personal information with partner insurance companies for users that submit applications for insurance. The information shared is the information provided by the user on the insurance application and supplementary forms. In all cases, the user has the opportunity to review the application and supplementary forms for completeness and accuracy. All forms are required to be physically or digitally signed by the applicant prior to submission to the insurance company. The shared information is used to process the user\’s request for an insurance product.

This transfer of information is a requirement for the insurance application process. If a user does not agree to this transfer, we will not be able to process the user\’s application request.

Partner insurance companies will use and retain the user\’s information as per their own privacy policies. TermLife.com does not control the privacy practices of our partner insurance companies.

Partner Web Sites

TermLife.com may occasionally work with partner Web sites to provide some insurance products. If a user requests an insurance quote supported by a partner Web site, the user will be directed to the partner\’s Powered By system for fulfillment. Information will be collected by the partner to process the user\’s request.

Any information collected is subsequently owned by the partner Web site and falls under the guidance of the partner Web site\’s Privacy Policy. TermLife.com does not control the privacy practices of our partner Web sites.

Service Providers

We partner with service providers to provide paramed exam services for certain types of insurance products. When a paramed exam is required by a partner insurance company, we provide the service provider with the user\’s contact information (such as name, address, and telephone number) that is necessary for them to provide their services.

This Web site contains links to other sites. Please be aware we are not responsible for the privacy practices of such other sites. We encourage our users to be aware when they leave our site and to read the privacy statements of each and every Web site that collects personally identifiable information. This privacy statement applies solely to information collected by this Web site.

Online Encryption

When our application forms ask users to enter sensitive information (such as social security number or medical information), the information is encrypted with and protected by the best encryption software in the industry – SSL (Secure Sockets Layer). The lock icon on the bottom of Web browsers such as Netscape Navigator and Microsoft Internet Explorer becomes locked while on secure pages, such as our application forms. Our Web site displays the VeriSign Secure Site Seal to show that our site is authentic and all transactions are secured by SSL.

HIPAA Compliance

The Department of Health and Human Services (HHS) published a final regulation called the Privacy Rule in December of 2000, in response to the Health Insurance Portability and Accountability Act (HIPAA) of 1996. The Privacy Rule became effective on April 14, 2003. The rule set national standards for the protection of consumers\’ Private Health Information (PHI) on the part of health plans and other health care providers that conduct business electronically.
